c.Letter ofMarriage/MarriageCertificatefor those who havemarried;
3.RIof the oldpassportto theapplicant'spassportreplacementRI;
4.Letter ofthe namechange(if itis planned tobea change orname change)
5.Written recommendationsfrom the leadershipfor those who workas civil servants, employeesof state enterprises,military/ policeorPrivateEmployees;
6.Applicantmake paymentin accordance withthe applicable(Government Regulation no.38 Year2009 regardingState Revenuein the Ministryof Justice andHuman RightsRepublic of Indonesia).
